Our highly skilled, clinical staff is dedicated to the treatment and recovery of individuals who have suffered: Stroke Trauma Neurological conditions including: Parkinson's Disease Multiple Sclerosis Guillian-Barre Syndrome Polyneuropathy Brain Injury Spinal Cord Injury Orthopedic Injury Amputation Medically Complex Illnesses including: Cardiac and Pulmonary, and Renal disease The Wound Care Nurse will function as part of the Wound Care program as a consultant to establish standards of care and clinical competence to meet the patients' needs. The Wound Care Nurse will assess, plan, implement, and evaluate the care of patients relative to their ostomy care, skin needs, and wound care needs. The Wound Care Nurse serves as a resource or consultant to patients, physicians, and team members for wounds, ostomies, incontinence, and actual or potential skin breakdown. Responsibilities: Provide consultation to all clinical areas for wounds, ostomies, incontinence, and actual or potential skin breakdown according to hospital policies and protocols. Leads the development and monitoring standards, guidelines, and pathways for care delivery of patients with wounds, ostomies, incontinence and actual or potential skin breakdown. Incorporates data and information to continuously improve care and practice to enhance clinical outcomes. Makes recommendations to the physician for changes to wound care orders and provides evidence based research as needed. Teaches staff and patients ostomy, wound, and pressure ulcer prevention through a variety of communication venues. Assess and document wound assessment data and communicate findings, revise/develop the plan of care and verbalize pertinent information to the health care team. Coordinates patient care in collaboration with other providers for the population served. Accurately performs patient assessments and identifies patient needs. Identifies and initiates appropriate nursing interventions. Provides patient and family teaching throughout the rehabilitation stay and prior to discharge. Provides care appropriate to condition and age of the patient. Works cooperatively and actively participates with other members of the interdisciplinary team. Participates in Continuous Quality and Performance Improvement (CQPI). May provide input into the budgetary planning process and contributes to cost effectiveness of services and programs provided by the department and hospital. Communicates employee observations and suggestions related to opportunities for improvement and staff educational needs. Maintains positive relationships with all customers. Complies with regulatory and accreditation standards. Responsible for personal and professional development. Maintains ethical practices and HIPAA requirements. Ensures compliance with policies and procedures regarding department operations, fire, safety, and infection control. Maintains medical staff rules/requirements, as well as regulatory body standards. Promotes rehabilitation nursing philosophy and scope of practice. Actively participates in team conferences. Performs timely and appropriate documentation relating to medical necessity in the medical record. Responsible for completion and revision of the Interdisciplinary Care Plan for each patient. Performs timely and accurate QI assessments upon admission and discharge for all patients. Sign IR-PAI upon discharge related to newly acquired or worsening wounds. Provides a safe environment for patients, visitors and staff and complies with appropriate and approved safety standards. Supports the leadership of the hospital in a professional manner and acts as a role model. Other duties as assigned. Experience/Skills: Associate's degree from Registered Nurse program, bachelor's preferred. Four to six years experience preferred.
