The LPN will function as a member of the Teamlet and/or support for specialty clinic(s) within the Community Based Outpatient Clinic. The Teamlet is comprised of the Primary Care Provider, RN, Clinical Associate and clerk. The LPN uses the nursing process and evidence -based practice to collaborate with the Teamlet and other Team members (family/caregiver, internal and community-based services involved in providing care to the patient) to develop the patient-driven holistic care plan for life.
