This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. You must meet specific individual eligibility requirements in accordance with VHA policy and submit your EDRP application within four months of appointment. Currently employed physician(s) in VA who met the requirements for appointment under the previous qualification standard at the time of their initial appointment are deemed to have met the basic requirements of the occupation. Basic Requirements: United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. Degree of doctor of medicine or an equivalent degree resulting from a course of education in medicine or osteopathic medicine. The degree must have been obtained from one of the schools approved by the Department of Veterans Affairs for the year in which the course of study was completed. Current, full and unrestricted license to practice medicine or surgery in a State, Territory, or Commonwealth of the United States, or in the District of Columbia. Residency Training: Physicians must have completed residency training, approved by the Secretary of Veterans Affairs in an accredited core specialty training program leading to eligibility for board certification. (NOTE: VA physicians involved in academic training programs may be required to be board certified for faculty status.) Approved residencies are: (1) Those approved by the Accreditation Council for Graduate Medical Education (ACGME), b) OR [(2) Those approved by the American Osteopathic Association (AOA),OR (3) Other residencies (non-US residency training programs followed by a minimum of five years of verified practice in the United States), which the local Medical Staff Executive Committee deems to have provided the applicant with appropriate professional training and believes has exposed the physician to an appropriate range of patient care experiences. Residents currently enrolled in ACGME/AOA accredited residency training programs and who would otherwise meet the basic requirements for appointment are eligible to be appointed as "Physician Resident Providers" (PRPs). PRPs must be fully licensed physicians (i.e., not a training license) and may only be appointed on an intermittent or fee-basis. PRPs are not considered independent practitioners and will not be privileged; rather, they are to have a "scope of practice" that allows them to perform certain restricted duties under supervision. Additionally, surgery residents in gap years may also be appointed as PRPs. Proficiency in spoken and written English. Duties include accessing the patient's medical record and clinical database as needed to permit clinical evaluation, utilization of TeleCritical Care software and audiovisual and written communication technologies, decision making and recommendations or orders and require the ability to perform a history and physical exam, review and interpret the medical record, form a differential diagnosis and clinical plan, make appropriate clinical decisions and provide recommendations, consultation and/or orders. Duties include the diagnosis and treatment of patients presenting with critical illness complaints, diseases, and injuries exclusively via telemedicine techniques from remote access. \"Diagnosis\" includes but is not limited to: ordering appropriate lab work, diagnostic tests and procedures, consultation to specialty services, synthesis of clinical data from multiple sources, assessment of these data and formulation of a differential diagnosis with determination of the most likely cause(s) of the patient's condition. \"Treatment\" includes but is not limited to: designing a management plan; providing consultation and recommendations; prescribing medications, ordering therapeutic procedures, and providing appropriate follow-up care. Duties may also include triage of patients with non-critical illness complaints, diseases, and injuries to appropriate disciplines for diagnosis and treatment, remote monitoring and management of devices such as pacemakers, chest tubes, ventilators including complex ventilator management, pulmonary artery catheter, central venous catheters, calibration and operation of hemodynamic recording systems and interpretation of hemodynamic waveforms and data. ICU telemedicine duties are understood to be performed under the direction of, or with the approval of the patient's bedside attending physician(s) as documented per memorandum of understanding between the East or West TeleCritical Care Program and remote VA facility receiving Critical Care telemedicine services. Maintenance of cordial and profession interactions between the monitoring center and ICU at all times is an essential attribute of the TeleCritical Care physician. The TeleCritical Care physician is responsible for contacting the bedside attending physician or his/her designee anytime physician presence is required at the bedside as delineated in the memorandum of understanding between the East or West TeleCritical Care Program and affiliated VA facility receiving Critical Care telemedicine services. The TeleCritical Care physician may participate and document end of life, resuscitation status, and clinical update discussions with patient's family as delineated in the memorandum of understanding between the East or West TeleCritical Care Program and affiliated VA facility receiving Critical Care telemedicine service. The attending physician at the bedside or his/her designee has final decisional authority for all patient care decisions.
