The Homeless Patient Aligned Care Team (HPACT) LPN works with a team of individuals to manage a panel of homeless patients in a primary care or specialty care setting. This position is part of the Community Resource and Referral Center (CRRC) under Social Work Service located at the NE Community Based Outpatient Clinic Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met.
