WHO WE ARE LOOKING FOR:

Franciscan Physician Network is seeking an experienced Family Medicine/Geriatric Nurse Practitioner to work full-time & join our PACE Program team at Franciscan Health Senior Health & Wellness in Indianapolis, IN to help meet the growing demands of our outpatient program.

WHAT WE NEED:

Ideal candidate will have nursing home care, family medicine and/or geriatric experience. Candidate would thrive in a fast-paced, autonomous, self-managed environment. Provider will provide direct medical care as needed to participants in the home, the PACE center, and in other health care settings. Provider will need to work collaboratively with our PACE Program team to coordinate and deliver the highest quality of patient care. Candidate will also have desire to develop strong ties with patients and the community in which they serve.

WHAT YOU CAN EXPECT:

• Practice Hours: Monday - Friday, 8:00 a.m. - 5:00 p.m.
• Flexible provider schedule
• Average outpatient volume: 4 - 6 per day
• Evening & Weekend Call: 1:2
• On-site training required in Indianapolis for up to 6 weeks
• Daily Duties:
  • Care planning with the PACE program's interdisciplinary team
  • Day-to-day care of patients
  • Review medical histories and medical assessments
  • Implement plan of care with patient and family
• Outreach potential with our Lafayette location
• Will work with skilled and long-term patients
