Department Overview
The Infectious Disease and Immunology department provides services relating to the diagnosis and treatment of all acute, chronic, and recurrent infectious diseases. They also provide vacination inforamtion, professional consultation, clinical support for community providers, and various educational and outreach programs.
Position Summary
This is a Case Management position with the MN Perinatal HIV Program, which works together with Children's HIV team in the care of newborns, and their families, who are perinatally exposed to HIV. This is a federally funded grant position, currently approved for one year, with possibility of extension. To provide family assessment, resource mobilization for families and children affected by HIV who have complex psycho-social and emotional needs in a multi-disciplinary pediatric hospital/clinic setting with frequent home visits. Pediatric case managers/social workers provide these services to HIV-exposed newborns and their siblings 0-18 years of age and their families. This position will work closely with the Perinatal HIV Nurse Coordinator and the Children's HIV Medical team including the HIV team social worker.

Minimum college or professional degree required. Bachelor's degree with 5 years experience in HIV community advocacy, case management, maternal/child care or home care. Master's degree in Social Work preferred. Knowledge of Community Resources. Experience providing crisis intervention. Two years experience with counseling in a health care related setting or two years experience in a pediatric related setting or completion of graduate internship in either of the above settings. Must have a working automobile. Mileage reimbursed based on federal rate. Must possess a valid driver's license (must meet the minimum criteria of Children's Minnesota Fleet Safety Management policy). Clean driving record. Must possess and provide verification of current automobile insurance coverage. Conduct face to face client appointments through home visits and appointments at external care sites (requires use of personal car and includes driving throughout the state of Minnesota).
