Franciscan Physician Network is seeking a Family Medicine Nurse Practitioner to meet the growing demand for primary care services at Franciscan Physician Network Heartland Internal Medicine located in Camby, Indiana. Provider will be joining an established, busy practice with physicians and advanced practice providers who enjoy a supportive work environment and a great work-life balance.

WHAT WE NEED:

Candidate would thrive in a fast-paced environment and be team oriented. The candidate will need to work collaboratively with physician to coordinate and deliver the highest quality of patient care. Candidate will also have a desire to develop strong ties with patients and the community in which they serve.

WHAT YOU CAN EXPECT:

• Flexible full-time schedule
• Weekday Call: Monday - Thursday
• Average outpatient volume: 18 - 21 patients per day
• Will see patient ages 13+
• Common Diagnoses:
  • Medicare wellness visits
  • Physicals
  • Diabetes and Endocrinology issues
  • Heart failure
  • Anxiety

Qualifications:

• Master's Degree Required
• Nurse Practitioner (NP) or Nationally Recognized Certification - Required
• Basic Life Support Program (BLS) - American Heart Association Required
