Atlantic Health System is seeking a Full Time Registered Nurse to join the Family Medicine department in Morristown, NJ. Monday through Friday, 9:00am-5:00pm. Rotating Saturdays as needed.
Responsibilities
Essential Functions:
The RN will work with and help complete reporting for value based programs such as CPC+, ACO and PCMH to complete quarterly objectives.
Will work with high risk patients to monitor and coordinate patients' treatment plans, educate them about their condition, connect them with health care providers, and evaluate their progress.
Perform transitional care management telephone calls for patients admitted to the hospital.
Occasionally perform all tasks associated with rooming patients including accurate taking and recording of vital signs.
Will be involved in population health management, performance improvement activities, and chronic disease management to meet quality performance goals for the practice.

Minimum Education:
Graduate of an accredited Nursing Program required
Education:
BSN preferred
License/Certifications:
Valid NJ Nursing License required
Relevant Experience:
1-2 years of experience as a Registered Nurse in an outpatient setting
