This position serves as a member of a VHA Patient Aligned Care Team (PACT), a patient-centered medical home model of care that is accessible, timely, coordinated, continuous, comprehensive, and compassionate. The PACT LPN completes assigned duties under the supervision of a Registered Nurse, using judgement in selecting the order and sequence of procedures and treatments, and recognizing, reporting, and recording relevant patient information. Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met.
