In this role, the APP will exercise autonomy in medical decision making and provide a broad range of diagnostic and therapeutic services as part of a multidisciplinary medical care team. Responsibilities: Conduct thorough physical examinations, assess patient histories, and diagnose common illnesses or conditions among the elderly patient population. Create, implement and regularly review care plans individually created for each patie
