Carle Health is seeking an Advanced Practice Provider to join our growing Internal Medicine team in Spring Valley/Henry, Illinois.

Opportunity Details

• Join an established practice with two Internists and experienced support staff
• Candidates with a strong interest in Geriatrics and/or Diabetes are encouraged to apply
• Position will be based out of the Carle Health Spring Valley and Henry clinics that offers Primary Care, x-ray (Henry) and laboratory services.
• Clinic hours are Monday, Wednesday 8 AM - 4:00 PM, Tuesday 7:30 AM – 5 PM, Thursday 7:30 AM to 4:30 PM and Friday 8 AM – 3 PM
• Fully integrated system with clinics and hospitals on Epic EMR to improve care coordination
• Carle Health is a not-for-profit organization with public service loan forgiveness eligibility

Education Requirements: 

Master's degree in Nursing or Physician Assistant

 

Licensure/Certification Requirements: 

Current Illinois registered nurse license and advanced practice nurse/physician assistant licensure

Compensation and Benefits

The compensation range for this position is $120,000 - $130,000. This represents a good faith minimum and maximum range for the role at the time of posting by Carle Health. The actual compensation offered a candidate will be dependent on a variety of factors including, but not limited to, the candidate?s experience, qualifications, location, training, licenses, shifts worked and compensation model.
