Registered Nurse - Tomah Clinic Ambulatory Care Float

The Ambulatory Care Float RN is responsible and accountable for all elements of the nursing process when providing direct Veteran care in the Patient Aligned Care Teams and Specialty Care Clinics using the nursing process and evidence-based practice to perform nursing functions related to coordination of patient care utilizing patient centered practices, and a focus on excellence in nursing care and customer service. The Ambulatory Care Float RN monitors and evaluates patient response to therapy based on Nursing Practice Standards, including monitoring lab results and other clinical data. She/he collaborates with patients to assess and identify needs, issues, care goals, and resources in achieving desired outcomes related to discharge,
